If the form is a Word document form and shows a square...if it was
properly created and protected, you SHOULD simply have to click in the
square with your mouse...or tab into the box and hit the spacebar to
cause the check mark to toggle on/off.
However, if the creator did not lock the document, then this feature
won't work correct. You can click Tools > Protect Document > Forms to
lock it and see if that gets the checkboxes moving. And then you
should also point the creator to my free forms articles so they learn
how to create forms properly. See this link:

However, if you are talking about an web page, then you'll need to
contact the web designer for more info.
Since you didn't provide many details as to whether you are talking
about a Word document, web site, or what the exactly the document is
telling you to do in its instructions...or even if YOU are trying to
test a form you've created yourself...it's hard to provide you with
more accurate details.
