Insert the chapter number and title in a header

In the cross-reference box the "for which heading" box is blank - nothing to
choose from. What am I doing wrong?

It would appear that your chapter number and title are not in one of Word's
built-in heading styles. Try using a StyleRef field instead. You'll need two
fields, one for the number and one for the text.
