Is this possible is Word? Forms question

I need to create a form for a client to fill out. The client would
like the form to be designed using Word. The Client would need to
select a team from a list of teams from a drop down list. Then based
on the selection, display a form specific form that team. Is this
doable in Word?

I think you need a UserForm (custom dialog box) for this. But a good
starting point would be Greg Maxey's article on how to "repeat data" in

Yes, it's rather easy to do. Lots of samples findable with google; try it.
