Logged in user gets locked out

I have a user on Vista logging into 2003 network. She can login successfully,
launch Outlook and be sending/reading emails etc, but then she may be
prompted to login to the Exchange server. Even though she types everything in
correctly she gets locked out.
Note: She is already logged in and may be sending and receiving emails when
this happens.

The server event logs only show her locked out, not the unsuccessful
attempts to login. The are set to be logged, because I see others failing to
login.

Any ideas on what to try?
