I hide markups then save my file then exit the application then when I open
the file again you can see markups again. How can I stop this from happening?

If you don't want to see the markup when you reopen the file, then it is not
sufficient to choose a view that doesn't show the markup. You must actually
accept all the changes.

The file is not prompting me to save any changes? Can I change some settings
to make it do this?

This has nothing to do with saving changes. You must *accept* the changes.
In Word 2002/2003, click the arrow beside the Accept Change button on the
Reviewing toolbar and select Accept All Changes in Document. Then save the
file.
