This sounds an awful lot like a homework question, which suggests that your
instructor has probably provided the answer in lectures or in the textbook.

No, it's not a homework question. I have to create a form using MS Word XP. I
have no idea where to find them. So, I thought if I asked it that way I would
then know what to look for.

tutorials cover protected forms, which are the most commonly used. The first
article covers ways to create forms that will be printed out and filled in
by hand or typewriter. In addition, you can add ActiveX controls from the
Controls Toolbox, but these are more suitable for Web forms or UserForms
