multiple address layouts?

Is it possible to have multiple address layouts?

ie on a template for an order form, when an address is chosen from the
addressbook to have it in a block with
company name
street address
city, state zip

etc etc....

(this i know how to do saving the info as a AddressLayout)

however, on another template, when an address is chose, put the
corresponding information in the appropriate areas?

ie i have a table in a word document, and in each cell of the table is the
corresponding info (ie phone, fax, attn, company, ect).

is there a way to have multiple address layouts for different documents
(templates)?

If you create a userform in each template that contains a combobox that you
populate with the names and addresses, then you can have code in the
userform, create document variables, the values of which are set to each
component of the address and you can then have the values displayed in the
document in whatever arrangement you desire simply by arranging the document
variables to suit.
