My Published form did not show up

I created a form as a second Details Tab on the contact form. (on OL97) Now I have a new computer with OL 2002 on it. I am able to see the contactsfrom the Public Folder but only with the original tabs - not the one I made. Where is it, is there a way to get it back?

Where did you publish the original form?

FYI, there is a separate newsgroup for Outlook forms issues -- microsoft.public.outlook.program_forms.
