My submit button on a sign up form takes me to "page not found"

As i enter enter a sign up form on a web page and loaded it to my provider
using Publisher 2003, i go to the web site and enter ingo on the for and
click submit and it gives me a page not found error. All the info on the form
properties is correct. The ISP tells me it's in the form. Can you help.
