Need help making a simple chart

All I want to do is make a simple chart with:
Score on the y-axis
Date on the x-axis
and have it show for one specific person.

I can not figure out how to do this. Seems simple enough but the wizard I
can not get it working to the way I want. I don't know how to set up a
manual chart either. It always wants a sum on the y-axis.

Can someone show me how to accomplish this or point me in the right direction?

Sure I just have a table called PHQ
It has SSN, PHQDate, and PHQTotal

these are the only fields that I need on the chart
I need it to just show for one SSN
Date on x-axis
PHQTotal on y-axis

Is this all you needed?
