Need Someone to Do Form

I have a 21-page application form in MS Word that I want people to be able to
fill in and then print out without changing the text of the form. I need this
functionality in MS Word and I also need a PDF version of the same form with
similar fill-in and print functionality. I don't need them to be able to send
either the Word or PDF back as a digital file. But it might be useful for
them to be able to save their work if they don't complete the form in one
sitting. I don't think I have time to learn how to do this. Can someone take
on this project? I'm not looking for a freebie. This is paid work, but my
client a non-profit not General Motors, so be friendly with the price.

I don't have the time to do it for you, others may. What you are talking
about is what Word calls an "online form." Check this in help. For more
