No Button after update

I'm trying to remotely debug a problem reported by a client who has upgraded
to Word 2007.

They had a template file with a vb button that launches a form.

They have security set to medium, which and they see the "do you want to run
macros" dialog, which they answer "yes" to.

This was working when they had 2003 installed, but I noticed that the screen
shot they sent showed the 2007 interface.

They assure me that they still get the run macro warning, but now the button
does not show up on the document. The screen shot shows this.

Is there any other new security settings that may have changed to cause this
behaviour? Is there anything in particular I should be looking for as as
cause?

Yes, they should check that the template is in a "Trusted location".

Office button, Word Options, Trust Center. Click "Trust Center Settings".
Select "Trusted locations" and click "Add new location". They should then
browse to the folder where the template is located.
