office xp keeps prompting for user name or initials

Whenever word or other xp app is opened, it asks for user
name and initials. It should do this on first use but
does it every time an office app is opened. Worse, it
also prompts on inserting pictures in powerpoint, causing
ppt to crash.
It's a win2k P III, office 2k was upgraded to office XP.
Tried uninstalling xp, rebooting, reinstalling to another
partition, still get name prompt. Saw nothing in MS
knowledgebase or google.
