Out of Office assistance

I have enable the "Out of Office Assiantance" in Ms Outlook 2003.
It works fine when internal user send email to me. But it is not working
when external user sent email to me (no auto reply to external sender).

What could be the causes?

The exchange server has out of office replies to the internet disabled.
