outlook 2002 forms design - Need help!

I am trying to create a form via the forms design button. I am able to
create a form in the Edit Compose Page, but when I click on the Edit Read
Page, my form is not formatted. I can save my form and email it to someone,
but when it comes back, the data is not there.
Need help!

After furious head scratching, Asylum asked:
