How do you delete a form from the Personal Forms Library?

i believe it is something like tools | options | other | advanced options |
custom forms | manage forms to get to the dialog that lets you remove a
form.

Thanks for the reply. I know I could have left the form
there, but I like to keep things neat and clean.
