Outlook contact layout has changed

The layout of the contact form has changed as follows:
Fisrt line of the form is now horizontal as opposed to the standard vertical
format. Also the order of information has changed. See below:

Phone number fields have remained the same; howerver, the order of the
address fields have been reversed:
