Outlook Contacts Importing Error: 1 Record per field

I am having trouble importing from an excel spreadsheet into my Outloo
contacts folder. After I setup the import and map the fields, Outloo
imports each field's value in as a separate record i.e. 1000 record
imported = 1000 contacts with only the name, 1000 contacts with onl
the first line of the address, 1000 records with only the City, you ge
the picture. I got the spreadsheet from a freind who was having the sam
problem. I have even tried exporting from Excel into a CSV file the
importing it into Outlook and get the same result. I am using Outloo
2003 SP1. Any help would be greatly appreciated

when it's in a CSV, what do the fields look like? Are they properly laid out
in the file, with comma or tab delimiters and each record on one line? (may
be wrapped... but still one line)

is the spreadsheet laid out with one complete record per row?

Yes there is one record per row and all the columns have header fields.
Here's a sample of the first couple of records in the CSV file:

Company,First Name,Last Name,E-mail address,Phone,Fax,Street 1,Street
2,City,State,Zip,Salutation
1960 Family Practice Center,Stephanie,Trocko,[email protected],+1 (281)

Any help would be greatly appreciated
