Please help make a drop down list that automatically goes to correct page

I am trying to make a drop-down list that people can use to redirect them to
the correct page.

here is what I have so far:
www.edreferral.com/searchtest.htm

does anyone know how to make this work? (hopefully without the submit
button..just automatically forward to the correct page)

any help appreciated.
