Problem with openning of an own form...

Hi,

in one of the public folders I have saved my own specific mail form.
In most cases when I open the form (file / new / choose form), everything
works fine.

But, sometimes when I try to open my form, it doesn't work. A default mail
form opens instead of my one!!!

Does anyone know the problem and the solution?
