Publishing to web

I have a sheet that i want to publish to the web. Included in the sheet is a
VBA form to input data to my sheet which opens automatically when the sheet
does. The problem i have is whilst the sheet uploads ok the form does not or
is not accessible!
Can ayone help me out.
