red x problem on 1st embedded graphic

Need help troubleshooting this problem
If I have a graphic embedded on an incoming message, that graphic
shows as a red X. If I have 2 graphics embedded on an incoming
message the 1st graphic shows a red x, but the second displays without
problems. If there are 3 graphics, the 1st is a red x the second and
3rd display properly. I've done a detect and repair, checked security
settings etc. Any other ideas on how to possibly troubleshoot this?
