What is the best format to save a scanned document as template in word to us a letter head.

..png for use in Word, I expect.

However, you really only want to scan logos, etc. and retype the letterhead
(assuming you have the proper fonts). See Suzanne Barnhill's site for
instructions on setting up a letterhead template.
