Scanning questionnaire document into excel/access

Is there a app within Microsoft Office or any other Microsoft software which
would enable me to scan about 100 x 2 page questionnaires and lift the data
from ticked/answered boxes which could then be exported to an excel or access
programme. We would then want to use that data for analysis and create
tables/graphs.

Probably only if you have OCR scanning capability, and that depending on the
OCR software, or complexity of the forms, may be problomatic, and no quicker
than entering data manually into excel/access (from which you could produce
tables/graphs)

Not easily. It would be easier and faster to pop the data into a database
manually.

My employer requires feedback surveys on a daily basis from our
patients,employees and visitors. Most of the forms are the same and once I
get them set up in Excel, populating the data by hand into the sheet does
not take long. Charts, graphs etc are already on the "template", I just
have to change the data source entry to make them all work. The only PITA
is the comment section.

Thanks for your quick response.
I thought Microsoft Document imaging was an OCR software bundled in Ofice
which had that capability of scanning documents. I thought I may be able to
create questionnaiires with tick/radio buttons then create an excel sheet on
exactly the same layout. By simply scanning a document it would drop the data
from the filled out boxes straight into the cells of the spreadsheet.
Is that wildly optimistic?

Optomistic, yes
Create the excel or access file, and enter manually, it will be far quicker,
and importantly, accurate

Yes.

But there *is* special hardware/software made to do exactly what you're asking
for. Schools use it to score standardized tests. I don't know any product names
or prices, though, just that it exists. Probably a lot pricier and more limiting
than hiring a kid to do the data entry for you. ;-)
