Select Unique Rows from Query to Place on Form

I have a query that produces 13 unique rows. What I
desire is to automatically filter the selection so that
certain values from the query are placed onto a form. For
example, I might want the value in the first row, third
column of the query to be placed in Text123 on my form.
Another example would be to take the average of the 13
values in column A and placed them in Textbox456. Please
help!
