Struggling with Forms in Front page 2000

Hello,
I'm a newbie to Frontpage and currently only have FP2000.
My client needs two forms for different types of visitors to complete
contact info, which will generate an email to a support person in my client's
company.
I am getting the same two problems on each form:

1. As soon as I publish the form [FrontPage Save Results Component] is
displayed at the top of the form - how do I get rid of it?

2. When the form is completed and submitted the following error message
appears:
FrontPage Run-Time Component Page
You have submitted a form or followed a link to a page that requires a web
server and the FrontPage Server Extensions to function properly.

This form or other FrontPage component will work correctly if you publish
this web to a web server that has the FrontPage Server Extensions installed.

My client's hosting company assures me that FP Server Extensions are
installed and working properly.
Is there anything I can do to test this? and (if they are installed and
working) what can I do to correct the problem?

are you publishing via FrontPage's http:// publish method? (file/ publish
web)?

If so, open the live http:// web with FrontPage (file/ open web, type in the
http:// address) then hit tools/ recalculate hyperlinks.

if it's still no go re-contact the host and have them reinstall your
extensions and make sure they're set up for email transport of form data,
since they're obviously not working.

If they say they did and they still don't work, it may be time to find
another host.
