Submitting web forms

I have created a questionnaire that I wil distribute to my listserv. I would like each person to fill out the questionnaire and when finished click the "Submit" button which will route the completed form back to my email address. My problem is figuring out how to configure the "Submit" button. Right now when you hit the submit button , a blank email will show up with my address as if to compose a message. What do I type in for "HTML Name" and "Action"
