Trouble inserting images

A few problems/questions:

1) How can I change the default view when I try to insert
an image? It is currently on thumbnail but I have so many
images that it takes forever for it to show the images to
insert. Very annoying to have to change it every time I
want to insert or change an image.

2) Even more annoying - At least once a session when I am
either go through folders looking for something, or even
just working with a file PP will lock up and I have to
restart the program. This will happen within a couple of
minutes of reopening the file. Around the fith or sixth
time it seems to stop for a while and work well.

3) Every so often the toolbars are not accesible. They
show up like normal I just can't click on the them. If I
minimize and then maximize the program I can access them
just fine. Very odd!

Any ideas on these problems??

As to the other problems, sounds very like a video driver issue of some
sort.
