What you would need to do is either recreate the form from scratch to
match the master..or, easier...add the image of the form to a document
as if it were a watermark. This will cause the image of the form to
appear in the background somewhat (in the header layer).
Then you can add form fields to the areas where you would fill in the
information. You would then click Tools > Protect Document > Forms so
that the document is locked...just allowing you to tab to each field
to enter the necessary information.
Note that it CAN be tricky getting the form fields into the proper
locations and then you'll want to double click the form fields to
limit the size so you don't type too much info that would readjust the
remaining fields to be off the mark with the form.
You can use tables to more precisely align the fields into the proper
locations.
Then when you print the form, I'll appear as if your information in
the form fields (in the upper layer) are printed within the form that
is under the text.
