Web page to initiates my project team?

I work in a new project team in a large organization. We would like to have
our customers fill out a list of simple questions via the web then my manager
be able to task and prioritize the tasks and if possible hook MS Project into
it for updates. Is there such a program out there?

Yes, take a look at SharePoint Portal Server. You can create a web site
with a survey web part that allows you to create requirements questions for
your users.
