accessing userform via menu item

I have a template which I am trying to allow the user to access a userform in
at will. I have placed a menu with specific options for the template on the
menu bar, but I'm struggling to access this userform by placing it as an
option on the customized menu.
Any help would be greatly appreciated!!!

userform.

Thanks Doug, I tried using a call statement and got an error, but this
confirms that is the way to do it, I just need to get my code right.
