Autolookup Query not working correctly

Okay, I need help for real.

I have created a database that has a bunch of tables, forms, queries,
reports, the whole nine yards. But here is what I need...

I have a table that has information for all 3000 of our firm members. I also
have a table for issues that they have when they call into our office. I want
to set up a form where the phone techs can enter the firms ID number and the
firms name and firm number (different from the ID number) show up along with
the other fields already on the issues table. I am trying to use the
autolookup query, and it works, but it is giving me all of the records
associated with that ID.

Essentially, I want the phone tech to go to a form, enter the firms ID,
check to see if the firm has called in before (I've used a subform for this)
and if they have not, I want the phone tech to click a button that opens a
new form (while closing the old form) and prepopulates the firm's (from the
first form) ID, Name, and Firm ID.

How do I do that? Let me know if you need more info. Thanks!
