Building a template or form

I need to create a form or template that I would be able to extract the
information entered out of the form or template field into a spreadsheet type
layout so that it can easily be imported into an access table.

Could word or excel give that kind of option?

Why not create the form in Access?

You can use Word and form fields for this as it has an option to save only
the data entered into the form fields in a CSV format. In order to provide
additional instructions we need to know the version of Office you are using.
