Cached mode not synching correctly

What things can cause the cached exchange mode to not sync correctly? The
user is remote from the SBS 2003 server, connected by a vpn and using Outlook
2003.

The inbox on the remote cached mode pc might indicate 55 messages in the
Inbox and all folders up to date. The exchange server (the real mailbox)
indicates there are only 42 messages.

We did a right click on Inbox folder, Properties, General tab, Clear
Offline Items to reset it, but at the end of that it had 28 messages in the
cached inbox with all folders up to date, but we know there are 42 messages
in the real mailbox.

A personal assistant on a different pc in the remote location has her
mailbox open and this users as well, and it shows the correct number of
messages in the Inbox.

I have previously deleted the whole ost file and recached the lot, but that
takes over 2 hours, and locks up the PA's Outlook for all that time. It must
think there are continual updates needed or something as the cache on the
other pc is refreshed. The end result was that it worked for a couple of
months, but is now out of synch again.

Any pointers to how to troubleshoot this would be appreciated. At the moment
we are having to use a direct connection to the mailbox without cached mode,
because we can not rely on it.
