Check boxes don't work on form

I have made a form to capture user input using MS Word
2002. The form uses numerous text fields and check
boxes. The text fields work great but the check boxes
are ignored. When I read the user response, I see the
text that went with the check box but I do not see the
check box itself. I do not even see any space where the
check box should have been. I thank you for any help you
can give.

Are these checkboxes and text form fields inserted from the Form toolbar as
used in a document protected for Forms?

Is the document protected?

If they are some other type of control, post back as describe where you got
them from.

If they are from the Forms toolbar, if you want to send the document to me I
will take a look at it.

Please respond to the newsgroups for the benefit of others who may be
interested.

Hi, Doug

Thank you for your prompt reply. I will send the
document to you. The document is a menu from which
customers will order a lunch.

The checkboxes and form fields are inserted on the page
by clicking the appropriate control on the Form Toolbar
and then clicking the form. After getting my form as I
wanted it, I clicked on the Protect button and then saved
the form as a template (.dot extension). I then opened
the form as a user would do, responded to the questions,
and then emailed the completed form to me as a customer
would do. When I opened the email, all of the
information that was entered into text boxes showed
perfectly.

There are several places where the user is to select one
or more choices from a list of choices by using the
checkbox control. In the email, I can see the text that
goes with the checkboxes but I do not see the checkboxes,
much less the response that was entered.

I am not sure what the difference is between "a document
protected for Forms" and "is the document protected".
Hopefully, the above will help you understand what I have
done and am trying to do. This is my first experience
with forms so please bear with me.
