Checkboxes in Forms

I'm creating a form using Word 2003. The form consists of about 10 questions
all of which the answers are Pass or Fail. My question is how do I get my
check box to only select one or the other and not be able to select both?

Although it is possible to achieve this with a macro, the whole point of
check boxes is to be toggles for two states: Yes/No, True/False, or, in this
case, Pass/Fail. A checked box would indicate a pass, an unchecked box
failure.
