Chekbox Value for Outlook Forms

I am attempting to make a customizable Outlook form with a checkbox.
want an email address to be auto populated with a certain email addres
when the checkbox is checked. I figured out how to do this with a radi
button by setting the radio button value to the email address, and the
making the "to" field equal to the radio button value, however I canno
set the value for a checkbox. Please help! is there code that I can us
behind the form to make the auto populate the "to" field with a certai
address once the checkbox is clicked
