create a data list by using a form

[Record keeping]

Is there a way in excel where I can take information from a templete and upon
saving it under a new file name have some of the feilds populate another work
sheet so I can have indivdual entries in the sheet after each update. Sort of
a form that keeps the answers to the calculations in the first sheet as a
unique record
