Creating a fill-in form

I'm trying to create a form that can be filled in on the
computer. I'm using form fields and I'm locking the
document but when I type text in the form fields,
everything else on the document moves too. What do I need
to do?

Use an unbordered table cell to contain the field and lock its height and
width. No more text can be entered than the cell will accommodate.
