Creating custom questionaire form/s in Frontpage

Can anyone tell me how to go about customizing the "Feedback" page in
Frontpage so I can include my own customized fields that the user would enter
info into?
IE: Guiding outfit: How many in party? etc.

Or instead of the Feedback form Template use the Forms Wizard to create your custom form

I was pretty successful by adding more fields on my form (via
copy-paste-rename; CPR). Just create the wizard form, then switch to HTML
view, and CPR where needed. Crude, but effective; especially if you are
adding just a couple of "simple fields". There is probably a better way to
do this, and if you find it, would you let me know also?
