Creating Forms w/out FP Extensions

To start with, my company has their web site hosted with an ISP that will not
use FP Extensions.

SO....

Can anyone tell me what the best method is to create a Form that I can have
a customer fill out online, then have the information sent to me in a usable
format?

I have created a Form in Word that will send me the info, but it is not in a
very usable format. I would like to be able to import the data so that a
similar form on my computer is automatically filled in.

Any help or thoughts would be great!!!!

You either have to use whatever server-side form handlers your hosting
service makes available, or program your own using ASP, PHP, CGI, or
whatever is available on the server.

Thank you Kevin,

I am not sure exactly how to gao about doing that, but I guess I will have
to learn. The first thing I guess I need to do is find out what is available
on the Server!

Depending of what they support, you can use ASP or PHP.
