Displaying check boxes and text next to it from a table

Hi! I have a work order and for that work order I have several things ti check.
Lik
1. check the mode
2.Clean the syste
3.Check the Internal Wirin

For each of these text above I need a "checkbox" next to it. For checking

All the above text must come from a table lik

TitleID TitleDescription Checkbox(Yes/No

I must able to add or delete the above items so that they will reflect on the form

Any help Appreciated
