How can I make the policy title appeared on the header of each page
automatically after the user filled in the policy title on the first page?

I have tried to use the text form, bookmark and cross-reference. It works
only when I select "Update Field" after the initial input. This document is a
template. It is intended for all the directors who created their department
policies.

If the REF field is in a header you will need to run an update macro on exit
from the field - the code you need is at
http://www.gmayor.com/installing_macro.htm.

REF fields in the body of the document will update if you flag the calculate
on exit property check box.

REF fields don't work in headers of protected documents. But you can apply a
specific style (paragraph or character) to the relevant form field and
insert a StyleRef field in the header. This will update automatically.
