make sure you have it just going to an e-mail address and
not also to a database as a txt or html file. also, do
not use a "mailto:" command, just insert the e-mail
address you want it to be sent to. you may have to
uninstall and reinstall the server extensions with your
hosting co.

and have the results emailed to me automatically. I have
designed the form and cannot figure out why it is not
