Email html form to non-Outlook customers

I want to email an html survey form to customers. They are to click a
couple of drop-downs and then click on the submit button to email the results
back to me. This works fine for customers using Outlook. But, won't work
for Lotus Notes customers.
Any ideas what approach I should be using? I want to email the form
directly because we believe the customer will be more likely to complete the
survey.

There is no way you will get that to work with Notes users.
Use an online form.
