Email Word from to Outlook doesn't retain check box

When I try to send a form "To Mail Recipient" (not as an attachment) the check boxes are not shown in the email. I can see the text information that I entered. Thank! LJD

This is normal behavior. Word forms don't translate well into HTML mail
messages (which is what you're creating when you use the Send To | Mail
Recipient command).
