Entering data to a worksheet using a form

Does anyone happen to know why I would get the error
message "cannot expand database" when trying to enter data
into an unprotected worksheet? I do not get this error
message when I enter the data directly, only when using
the form option found in the Data Menu. Any help would be
greatly appreciated. Thanks.

The workbook may have a named range "Database" that is incorrectly
defined. To correct this:

1. Choose Insert>Name>Define
2. Select the name 'Database'
3. Change the formula in the 'Refers to' box, to match the range on the
worksheet.
4. Click OK

Or use a dynamic formula to define a range named database. There are
