Entry field needs to stay underlined/lined when text entered.

I have a document with multiple values to be entered on it. For example:

Version: _______________ Q-Item: ____________________
PL #: _______________ Date: ____________________

I need the lines/underline above to always appear below the entered text. I
also need each field to only allow a fixed number of characters. And I need
the characters enterered to not push out the field next to it. For example:

Version: __test_________ Q-Item: ____________________
PL #: 12345678901234 Date:
____________________

All above is correctly shown but 'test' and the numbers would have a
line/underline under them.

I can do every item needed easily by itself but not all together. I'm sure
there is an easy answer, but I'm tired of looking for it.
