ER diagram

Could anyone tell me how to setup the functionality in Visio to create a standard ER diagram with entities, attributes, and relationships? I don't want to see the attributes within the entity as in a database diagram, but rather as ovals connected to the entiry. I don't see this kind of functionality built into any of the templates, but I'm sure that I'm missing something obvious

You need the Visio Professional edition or Visual Studio Enterprise
Architect edition of Visio. ER diagramming is not found in Visio Standard
(or older Visio Technical editions).

Otherwise, you'll have to draw them manually (create your own stencils and
templates).

The Chen ER notation (i.e. Attributes encircled outside of Entity boxes) was
supported by, I think, Visio 5.0. You might be able to find the stencil
somewhere. However, that stencil was just that -- a stencil. It would not
generate the database, DDL, reverse engineer, and etc. For that, you will
need Visio for Enterprise architect (shipped with Visual Studio .Net
Enterprise Architect).
