I'm unsure of whether there's something you could buy for the server and do
it "cleanly", but I have gotten this to work with just a FrontPage form.
Using outlook 2002, and an app called, I think "hotfax" from smith micro,
you can set up your form to deliver the email results to a specific email
box, then use the "rules wizard" in outlook to forward by fax, certain ones
that fall within a parameter.
you can set up a hidden field in your form to contain something like the
user's name or a code to identify him:
##someguy1##, then the rules wizard would catch the ##someguy1## and forward
it by fax to a phone number (that's where the smith micro app comes in).
It worked OK, didn't cost too much to put together either.
